Pursuant to Bank Negara Malaysia’s Guidelines on Credit Transactions and Exposures with Connected Parties/Bank Negara Malaysia’s Guidelines on Credit Transactions and Exposures with Connected Parties for Islamic Banks and Public Bank Group's Credit Policy on Credit Facilities and Exposures to Connected Parties on granting of credit/financing facilities by Public Bank Group (hereinafter referred to as “the Bank”) to the following connected parties:

Director of the Bank;
Controlling shareholder of the Bank;
Influential shareholder of the Bank;
Executive officer of the Bank;
Officer of the Bank who is responsible for or has the authority to appraise and/or approve credit transactions or review the status of existing credit transactions, either as a member of a committee or individually;
Close relatives including dependents*, spouse, dependents of spouse, child (including step child and adopted child), spouse of child, parent and brother or sister and their spouse of the persons referred to in items (i) to (v) above;

* Dependents refer to any person who is financially dependent on the individual or his spouse for his livelihood, e.g. a person who receives financial assistance on a regular basis from the individual/spouse.
Firms/partnerships/companies/legal entities which control, or are controlled by, any of the persons listed in items (i) to (vi) above;
Firms/partnerships/companies/legal entities in which any of the persons listed in items (i) to (vi) above is interested as a director, partner, executive officer, agent or guarantor, and their subsidiaries or entities controlled by them;
Any person for whom any of the persons listed in items (i) to (vi) above is a guarantor.
Subsidiary of, or an entity controlled by, the Bank, and its connected parties.

I/We hereby declare that:

I/We, Individual Guarantor(s)/Corporate Guarantor(s) and Chargor(s)/Assignor(s)/Pledgor(s) are not connected party as listed above.
I/We hereby undertake to inform you immediately in the event that any such relationship as stated above arises in future.
I/We agree that the Bank reserves the right to terminate/recall the facility(ies) granted in the event I/We have failed to make the appropriate or correct declaration.
